Qualifications

• Active Oklahoma CNA licensure/certification.

Responsibilities

• Provide personal care (baths, grooming, hygiene) and assist with activities of daily living.
• Assist with dressing, grooming, and meal planning/feeding as needed.
• Monitor and report patient observations; document care performed.
• Record vital signs and maintain accurate records per plan of care.
• Support therapy activities and ambulation as directed.
• Attend required in-service training and IDG meetings.
