Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care to patients in their homes according to a care plan.

Responsibilities include assisting with bathing, dressing, mobility, hygiene, feeding, and documenting care while maintaining safety and privacy. CPR certification and state-required training are required; the team emphasizes quality, compassion, and reliability.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
• Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
• Safe transfer and ambulation techniques.
• Exercise according to the plan of care.
• Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
• Patient’s laundry.
• Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Observation and teaching of home safety practices.
• Recognition of an emergency situation and knowledge of emergency management procedures.
• Must follow patients’ plan of care and complete all tasks assigned.
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office.
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor.
