Job summary

The Lakes Home Care seeks a compassionate Home Health Aide to provide daily living and personal care in patients' homes per an established care plan. You'll assist with baths, grooming, mobility, meals, and daily activities, documenting care and communicating with the supervising nurse.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintain CPR certification.
• Participation in Maxim screening and competency program (if applicable).

Responsibilities

• Assist patients with baths, grooming, dressing, toileting, feeding, and daily living tasks.
• Take and record vital signs as ordered (temperature, pulse, respiration, blood pressure).
• Assist with safe transfers and ambulation and support mobility as per care plan.
• Perform activities per plan of care and document care provided.
• Communicate clearly with client, family/caregiver and supervisor; observe and report changes.
