Job summary

Advocate Health in Union region seeks a Speech Language Pathologist to lead home health speech therapy services for a defined group of patients. You will provide direct care per a plan of care and physician orders, focusing on patient goals and safe outcomes.

The role requires a Master's in Speech Language Pathology with current state licensure and BLS certification, and you will collaborate with a multidisciplinary team to coordinate care in patients' homes.

Qualifications

• Master's Degree in Speech Language Pathology and current state licensure required.
• BLS required per policy guidelines.

Responsibilities

• Manage daily patient caseload and ensure timely care and documentation per plan of care and MD orders.
• Collect data from assessments in the home setting, telephonic contacts and EMR.
• Evaluate patients to determine rehab needs and develop a plan of care with the physician.
• Implement plan of care and provide ST services certified by the physician.
• Direct care coordination of a multidisciplinary clinical team to promote health and safe outcomes.
• Maintain communication with physician oversight and branch supervision for timely feedback.
• Perform treatments and procedures in accordance with licensure and guidelines.
