Job summary

Avera is seeking a Speech Language Pathologist to evaluate patients, create treatment plans, and provide therapy in a home-health setting. The role involves assessing deficits in speech, language, cognition, voice, and dysphagia, and coordinating care across rehabilitation, medical, and nursing teams.

Eligible clinicians may receive a sign-on bonus up to $20,000 and may participate in the Student Loan Repayment Program. Excellent teamwork and compassionate care are emphasized.

Qualifications

• Must hold CCC-SLP certification or be actively pursuing supervised fellowship.
• Active state licensure in speech-language pathology required.
• Ability to communicate effectively with patients, families and team.

Responsibilities

• Evaluates patients and interprets diagnostic assessments to determine intervention needs.
• Develops and implements individualized speech therapy programs.
• Plans and coordinates treatment with rehab, medical, and nursing teams.
• Documents evaluation results and treatment progress in patient records.
• Communicates patient status and goals with the care team and family.
