Job summary

Visiting Rehab and Nursing Services is seeking a Part-Time Physical Therapist (PT) to join our home care team in Mashpee, MA. You will provide skilled PT services in patients' homes, promoting mobility, safety and independence with flexible scheduling and competitive per-visit pay up to $160.

Qualifications

• Current Massachusetts Physical Therapist license is required.
• Minimum of one year of clinical experience.
• Afternoon availability.

Responsibilities

• Providing comprehensive physical therapy evaluations
• Developing individualized treatment plans based on each patient’s needs
• Providing therapeutic exercises and functional mobility training
• Assessing patients’ home environments and recommending appropriate safety modifications
• Educating patients and caregivers
• Monitoring patient progress and modifying treatment plans as needed
• Collaborating with physicians and other members of the care team
• Completing accurate and timely electronic documentation
