Job summary
Visiting Rehab and Nursing Services is seeking a compassionate Per Diem Physical Therapist (PT) to join our home care team in Lenox, MA. You will provide skilled PT services directly in patients' homes, promoting mobility, safety and quality of life with flexible scheduling and competitive compensation.
Requirements include MA PT license, at least 1 year of clinical PT experience, and afternoon availability.
Qualifications
- Massachusetts PT license required.
- At least 1 year of clinical PT experience.
- Afternoon availability required.
Responsibilities
- Perform home-based PT evaluations.
- Develop and implement individualized treatment plans.
- Provide therapeutic exercises and mobility training.
- Assess home environments for safety and recommend modifications.
- Educate patients and caregivers.
- Monitor progress and adjust treatment plans as needed.
- Collaborate with physicians and care team.
- Complete accurate electronic documentation.
