Job summary

Valor Healthcare is seeking a Health & Wellness Coach for the Community Based Outpatient Clinic (CBOC) in Okeechobee, part-time (3x/week). The role provides prescribed medical treatment and personal care, collaborating with the PACT team to develop patient care plans and support chronic disease management.

Ideal candidate holds NBHWC certification (or is eligible), completes NBHWC training, and has completed 50 health and wellness coaching sessions, with an associate degree or 4,000 hours of

Qualifications

• Must hold NBHWC certification or be eligible to sit for the exam.
• Complete an NBHWC approved training program.
• Complete 50 health and wellness coaching sessions; and
• Have an associate degree or higher or 4,000 hours work experience (any field).
• Ability to emphasis and use judgement on health enhancements/strengths, rather than disorders/weaknesses.
• Adaptive learner and having the willingness to acquire knowledge on new technologies to solve problems.
• Excellent problem solving and analytical skills.
• Ability to work independently, self-directed, and solutions-oriented.
• Possesses sound organizational and planning skills with a solid mindfulness.

Responsibilities

• Provide prescribed medical treatment and personal care services to patients.
• Collaborate with the core PACT Team and expanded PACT Team to develop patient care plans.
• Support clients in mobilizing internal strengths and external resources and develop self-management strategies for healthy lifestyle changes.
• Provide guidance and resources from recognized authorities to contribute to patients’ wellness.
• Coach patients toward healthy lifestyle changes using goal setting and accountability.
• Assist patients with weight management, smoking cessation, and proactive health components.
• Provide care through telephone clinics or secure messaging between appointments.
• Coordinate care with primary and specialty care providers for chronic illness management.
• Engage patients in personal health planning through individual and group programs.
• Educate external stakeholders about Whole Health Group and programs.
