Job summary

Building One Community in Stamford, CT, seeks a part-time Community Support Coordinator (25 hours/week) to connect immigrant families with health resources, focusing on Healthy Steps, health navigation, and mental health initiatives. On-site role with some evenings/weekends, reports to Family Services Manager.

Responsibilities include health assessments, care planning, data management in Salesforce, and collaboration with health partners. Fluency in English and Spanish required.

Qualifications

• Fluency in English and Spanish required.
• Minimum 2 years of experience in community health or public health with immigrant or underserved populations.
• Bachelor's degree in Public Health or related field required.

Responsibilities

• Conduct health and basic needs assessments to identify gaps in care and access.
• Develop personalized care or resource plans with participants' goals and social needs.
• Coordinate and deliver the Healthy Steps program and Mental Health Initiative; connect to resources.
• Provide health education on preventive care, nutrition, and chronic disease management, with culturally appropriate materials.
• Assist participants in understanding insurance coverage, billing and financial assistance.
• Maintain up-to-date case files; enter data in Salesforce; generate reports and analyze outcomes.
• Collaborate with Family Services and Community Engagement teams to expand health-provider connections.
• Coordinate and support onsite health events with local providers.
• Supervise volunteers supporting health programs and events.
