Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join an integrated clinical team on a part-time basis, working two days weekly onsite in local long-term care facilities. You will deliver therapy, assessments, care planning, and behavioral health support with strong administrative and clinical back-up.

The role offers autonomy within an interdisciplinary setting, with a predictable schedule and opportunities for professional growth and CEUs.

Qualifications

• Active, unrestricted LPC license is required.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with a team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Coordinate with interdisciplinary teams to optimize resident outcomes.
• Maintain documentation using the Geriatric Notes Tool (GNT).
• Support residents with common LTC concerns (depression, anxiety, grief, etc.).
