Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join our Long-Term Care Clinical Services team on a part-time, on-site basis. You will provide therapy, assessments, care planning, and behavioral health support to residents in LTC facilities within your community, supported by administrative and clinical resources.

Qualifications

• Active, unrestricted LPC license.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment disorders, grief, trauma, and other LTC-related needs.
• Deliver behavioral management and brief, goal-oriented interventions.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate treatment.
• Participate in care plan discussions and facility meetings.
• Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
• Follow ethical, professional, and regulatory guidelines for clinical documentation and care delivery.
• Build strong relationships with facility staff and leadership as a behavioral health resource.
