Job summary

Harbor Health is seeking a part-time Chaplain to join the Elder Service Plan (PACE) team in Brockton, MA. The Chaplain will provide spiritual leadership, counseling, and religious program administration under the ESP Social Services Manager, supporting participants, caregivers, and families.

Responsibilities include organizational worship and education, collaboration with IDT, documentation in the EMR, and bi-weekly prayer visits.

Qualifications

• Undergraduate degree and a Master of Divinity from an accredited seminary or theology school.
• At least two years of pastoral/spiritual leadership in a non-profit setting.
• Contextual education may satisfy one year of experience.
• Computer literacy.
• Proven organizational skills.
• Proven oral and written communication skills.
• Experience working with a geriatric population preferred.
• Travel between Harbor ESP sites and occasional home visits as needed.

Responsibilities

• Provides spiritual leadership, counseling, and spiritual assessments for participants and caregivers.
• Participates in IDT meetings and care planning as appropriate.
• Maintains timely documentation in the electronic medical record.
• Provides Christian education and spiritual support.
• Supports participants through bi-weekly prayer and visits.
• Travel to Boston and Brockton locations and home visits as needed.
• Hosts sessions on contemplative practices, happiness maintenance, grief and loss, forgiveness, and healthy relationships.
