Job summary

Harbor Health Services, Inc. is seeking a part-time ESP Chaplain for the Elder Service Plan (PACE) in Brockton, MA. The role provides spiritual leadership, counseling, and program administration to participants and families.

The chaplain will collaborate with IDT, participate in care planning, document care in EMR, and travel between Brockton and Boston locations as needed. Requires a Master of Divinity and experience in geriatric settings. Benefits are offered.

Qualifications

• Undergraduate Degree and a Master of Divinity from an accredited seminary or theology school (three-year education).
• At least two years of pastoral/spiritual leadership in a non-congregation or nonprofit setting.
• Contextual education training may satisfy one year of experience.

Responsibilities

• Provides spiritual leadership, counseling, and religious program administration.
• Participates in IDT meetings and care planning.
• Maintains timely documentation in the electronic medical record.
• Provides Christian education and spiritual support to participants and caregivers.
• Supports participants through bi-weekly prayer and home visits.
• Travels between Harbor ESP sites and to participant homes as needed.
• Hosts sessions on contemplative practices, grief, forgiveness, and relationships.
• Collaborates with IDT and Social Services Manager to provide comprehensive spiritual care.
• Attends ethics meetings and trainings; works with clergy of other faiths; administers sacraments and last rites.
