The ideal candidate has 1+ year APP experience, with preferred background in FQHC/community health settings and post-masters training in Community Health Care.

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center would be beneficial.
• Post-masters training in Community Health Care would be of great benefit to the practice.

Responsibilities

• Treats family medicine, pediatrics, and adults with complex issues.
• Shared call schedule with RN first call.
• Co-manages patients with a diverse care team to improve holistic health.
• Reviews quality and outcome metrics with the healthcare team monthly.
• Collaborates with local civic and faith-based leaders on community education programs.
