Qualifications

• Must have high school diploma or GED.

Responsibilities

• Provide direct care and support to individuals in homes and in the community.
• Assist with activities of daily living, including hygiene, meals, light tasks, and skill-building.
• Encourage social engagement and community integration through activities.
• Follow individualized support plans and goals for each person.
• Maintain documentation of daily activities, progress notes, and incidents.
