Job summary

Premise Health in South Bend, IN is seeking a Part-Time Dietitian to provide nutrition care to employees and dependents at our Health Center. The role supports a multidisciplinary team and involves comprehensive assessments, risk reduction recommendations, and care planning.

The position begins early January 2027. Responsibilities include education, program planning, and potential collaboration with cafeteria vendors.

Qualifications

• Bachelor’s degree with course work approved by the Academy of Nutrition and Dietetics.
• Master’s degree preferred.
• State license/registration as applicable (e.g. copy of license or verification).
• CDR certificate number & certificate card.
• State Lab Certificate (NV only).
• Additional certifications in wellness/fitness preferred.

Responsibilities

• Collaborates with multidisciplinary team to deliver optimum member care.
• Conducts a comprehensive nutritional assessment on members ages 2 and up, including nutrition history, risk factors, labs, and medical history.
• Identifies areas to decrease health risks and provides evidence-based recommendations and referrals as needed.
• Develops individualized care plans and may assist members in creating SMART goals while evaluating progress.
• Documents nutrition care plans and member progress in medical records.
