Job summary

Premise Health is seeking a Full Time Registered Dietitian to provide nutrition care for employees and dependents at our Countryside, IL Health Center. You will conduct comprehensive nutrition assessments for members ages 2 and up, deliver evidence-based guidance, and develop personalized plans with SMART goals.

Collaborating with a multidisciplinary team, you will review HEDIS-related care for diabetes or HTN, plan wellness programs, and document progress in medical records.

Qualifications

• Bachelor’s degree in Nutrition/Dietetics; Masters preferred.
• State license/registration required.
• CDR certification with active CDR number.
• Three years as a registered dietitian in a clinical setting.
• Experience in health promotion or work-site wellness.
• Public speaking and presentations experience.

Responsibilities

• Collaborates with multidisciplinary team to deliver client member care.
• Conducts comprehensive nutritional assessments for members aged 2+.
• Identifies risk factors and provides evidence-based recommendations.
• Develops individualized care plans and SMART goals with progress follow-up.
• Documents care plans and progress in members' medical records.
• Reviews diabetes and hypertension care standards and outcomes.
• Plans and presents nutrition wellness programs; may market and evaluate them.
