Qualifikationen

• Active Pharmacy license in Illinois.
• 3 years of experience in a clinical pharmacy practice.
• Immunization certification or willingness to become immunization certified.
• Certification in geriatrics and/or gerontology preferred.

Aufgaben

• Ensure facilities remain compliant with federal regulations.
• Complete clinical reviews as required by federal and state regulation.
• Own issue resolution and communicate early warning signs of potential issues.
• Drive adoption of Omnicare programs and service offerings.
• Use existing customer relationships to promote growth.
• Conduct Executive Reviews and other customer meetings.
• Collaborate with Account Management, Sales, Operations, and corporate functions to meet site goals.
• Perform Medication Regimen Reviews (MRR) and provide written reports.
• Spend appropriate time on each resident's review per clinical needs.
• Use OSC2OR on-site to generate MRR and recommendations.
• Utilize MDS 3.0 health assessment tool for targeted residents.
• Complete Quality Improvement reports and monthly summaries.
• Educate facility staff when new programs are implemented.
