Job summary

Horizon House, Inc. is seeking a part-time Community Support Counselor for our Susquehanna Park location to help individuals access the services they need to live, learn, work and socialize in the local community.

You will provide ongoing support under the Program Director’s guidance, engage with clients in community settings, and help maintain continuity of care while coordinating local travel as required.

Qualifications

• High School diploma or GED required.
• Valid driver's license with good driving record required.
• Ability to work independently and as part of a focused team.

Responsibilities

• To go into the community to provide assistance to individuals with services they need to live, learn, work and socialize in the local community.
• To provide support to individuals and assure continuity of care, under the immediate guidance of the Program Director.
