Job summary

Horizon House, Inc. is seeking a part-time Community Support Counselor for our Susquehanna Park location to help individuals access the services they need to live, learn, work and socialize in the local community.

You will provide ongoing support under the Program Director’s guidance, engage with clients in community settings, and help maintain continuity of care while coordinating local travel as required.

Qualifications

• High School diploma or GED required.
• Valid driver's license with good driving record required.
• Ability to work independently and as part of a focused team.

Purpose:

• To provide support to individuals and assure continuity of care, under the immediate guidance of the Program Director

Requirements:

• Valid driver’s license with good driving record required.
• Ability to work independently and as part of a focused team.

Scope:

• Frequent talking and listening.
• Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.
• Must be available for local travel.
• Possess manual dexterity and fine motor skills.
• May require crisis intervention to emergency situations.
• Must be available for scheduled emergency on-call service.
