Qualifications

• Experience in community outreach or referral development preferred.
• Reliable transportation and a valid driver's license required.
• Willingness to travel locally to referral sources and events.
• Excellent verbal and written communication skills.

Responsibilities

• Represent the agency in the community and build referral relationships.
• Visit healthcare and community organizations to introduce services.
• Identify contacts and maintain ongoing relationships with sources.
• Research events like health fairs and senior expos for presence.
• Coordinate events, set up informational booths, and distribute materials.
• Follow up by phone, email, or in person with leads.
• Provide weekly outreach reports to the Administrator.
