Job summary

Community Options, Inc. is hiring a Part-Time Community Integration Counselor in Allentown, PA. The CIC will assist individuals with learning life skills, support community integration, and document progress toward independence.

Responsibilities include coordinating activities, transporting participants to appointments, and ensuring accurate documentation. Minimum requirements include a high school diploma or GED (Bachelor’s preferred), a valid driver’s license, and at least two years of

Qualifications

• High school diploma or GED; Bachelor’s degree preferred.
• Valid driver’s license with satisfactory driving record.
• Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities.

Responsibilities

• Support and encourage individuals to increase independence.
• Meet waiver participants regularly in residence, community, or office settings.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration.
• Assist in building a support network with the community.
• Administer medications and ensure proper documentation.
• Coordinate leisure and volunteer opportunities.
• Monitor health and medical needs and report concerns.
• Drive individuals to medical appointments and activities.
• Ensure program documentation and billable records are completed accurately.
