Job summary

Community Options, Inc. is seeking a part-time Community Integration Counselor in Williamsport, PA. The role focuses on helping individuals learn life skills and promoting independence, with a pay rate of $16.75 per hour.

Workdays are Monday through Friday, with potential evenings or weekends depending on needs. Responsibilities include meeting participants in various settings, collaborating with the Case Manager to design supportive programs, documenting progress, coordinating leisure and

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Minimum 2 years of experience supporting individuals with intellectual and developmental disabilities.
• Valid driver's license with satisfactory driving record.
• Excellent verbal and written communication skills.
• Ability to establish a supportive relationship and exercise good judgment in crises.

Responsibilities

• Meet waiver participants in residence, community, or office to support independence.
• Collaborate with Case Manager to develop programs for social, emotional, physical and personal needs.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration.
• Coordinate leisure and volunteer opportunities.
• Administer medications and ensure proper documentation.
• Drive participants to medical appointments and activities.
• Complete program documentation and billable records timely.
