The starting time for this position is varies based on availability**GENERAL SUMMARY:**Verifies eligibility for the program using both income screening and health screening techniques. Enters data and maintains accurate medical records and charts for individuals. Schedules appointments for all clients and assist with any questions or concerns. Provides training to clients in regards to using the program and accessing their benefits, education opportunities and referrals.
