Southwest Human Development, Incorporated. is seeking a part-time Community Health Nurse to provide home-based developmental and health follow-up for infants and toddlers, including NICU graduates.

The role emphasizes family-centered care, early intervention coordination, and education for parents and caregivers. The position involves in-home and virtual visits, documentation in an EMR, and collaboration with healthcare providers and community partners to support healthy child development.

Qualifications

• Experience in home visiting, neonatal, pediatrics, and/or maternal-child preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct in-home and virtual visits with families to monitor child health, development, and family well-being.
• Build supportive relationships with parents and caregivers while providing education, guidance, and encouragement.
• Complete developmental and family wellness screenings using standardized assessment tools.
• Identify developmental, medical, or social concerns and coordinate referrals to early intervention and community resources.
• Support positive parent-child interactions and help families achieve health and developmental goals.
• Collaborate with healthcare providers, school districts, early intervention programs, and community agencies to coordinate services.
• Maintain an active caseload and provide ongoing follow-up from enrollment through program discharge.
• Schedule visits, communicate with families, and provide reminders to support engagement in services.
• Complete accurate and timely documentation, assessments, case notes, and reports in the electronic medical record system.
• Contribute to continuous quality improvement initiatives and ensure compliance with program, agency, and funding requirements.
• Maintain confidentiality and uphold HIPAA, safety, and infection control standards during all client interactions.
