Job summary

Texas Children's Hospital is seeking a Nurse Home Visitor for the Nurse Family Partnership. The role provides in-person case management for high-risk mothers and infants, coordinating care to improve health outcomes and reduce costs.

Responsibilities include assessing needs, planning care, and collaborating with health plans, providers, and families. A strong background in women’s health and home-visiting experience is preferred, with bilingual English/Spanish highly desired.

Qualifications

• Required bachelor’s degree in nursing.
• RN licensure in Texas or Nursing Licensure Compact.
• Driver's license and current auto insurance.
• BLS certification by American Heart Association.
• Minimum 3 years clinical or public health experience, preferably Maternal-Child Nursing.

Responsibilities

• Assess, develop, implement, and monitor a comprehensive care plan for mothers and babies.
• Coordinate services across continuum of care with team and family.
• Document plans and progress in designated systems.
• Advocate for members and families to ensure continuity of care.
