Job summary

Unity Care NW is hiring a Communicable Disease Program Coordinator to provide administrative support and help coordinate all aspects of the Communicable Disease program at our clinics in Whatcom County. This on-site role focuses on patient, health center, and community program coordination with cross-department collaboration.

The ideal candidate has 3+ years in social services or public health, strong communication skills, and experience with EHR and CQI initiatives.

Qualifications

• 3 years’ experience in social services or public health or equivalent education, training and experience.
• Basic written and verbal communication, computational and computer skills.
• Knowledge in public and/or community health from certificate or license program.
• Experience coordinating with a medical clinic and leading multi-disciplinary teams.
• Program coordination experience.
• Ability to analyze and report clinical outcomes and participate in CQI initiatives.
• Sensitivity to diverse populations.
• Proficiency with Word, Excel, Outlook, SharePoint, Forms & PowerPoint.
• Learn and use EHR, scheduling, HR/Payroll systems, survey software, and LMS.

Responsibilities

• Coordinate all aspects of the Communicable Disease program at UCNW.
• Provide administrative support and assist with patient, health center, and community programs.

• Support continuous quality improvement initiatives.
