Job summary

Unity Care NW is seeking a Part-time Communicable Disease Program Coordinator to provide administrative support and coordinate all aspects of the Communicable Disease program at UCNW. The role is on-site in Whatcom County, WA, with a focus on patient, health center, and community program coordination.

Requirements include 3 years in social services or public health, strong communication skills, and experience leading multi-disciplinary teams within a medical clinic.

Qualifications

• 3 years’ experience in social services or public health.
• Possesses a basic level of written and verbal communications skills.
• Experience leading multi-disciplinary teams within a medical clinic.
• Ability to analyze and report clinical outcomes and participate in CQI initiatives.
• Familiarity with EHR, scheduling, HR/payroll systems.
• Ability to work cross-departmentally with outside agencies.

Responsibilities

• Coordinate all aspects of the Communicable Disease program.
• Provide administrative support to patient, health center, and community programs.
• Prepare reports on clinical outcomes for CQI initiatives.
• Collaborate with cross-functional teams and external partners.
