Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Ability to read, write and follow directions.
• Ability to lift and assist with transfers as needed.

Responsibilities

• Provide direct patient care as assigned.
• Assist with transfers, repositioning, and ambulation.
• Provide hygiene care and light meals per plan.
• Offer companionship and activities tailored to client needs.
• Record changes and maintain required documentation.
• Follow Plan of Care and EVV task/timesheet requirements.
• Report concerns and ensure patient confidentiality.
