Job summary
Safe Life Home Health Care is seeking dedicated CNAs to join our Bartlett, IL team. You will assist home health patients with daily activities, monitor vital signs, and provide compassionate, safe care in patients' homes.
This role emphasizes communication with families and the healthcare team, along with accurate documentation in the EMR system. Responsibilities include ADLs, mobility support, medication reminders, and nutrition assistance, while maintaining infection control and safety
Qualifications
- Active CNA certificate required.
- CPR certification required.
- Valid driver’s license and auto insurance required.
- Prior home health experience is a plus.
- Familiarity with EMR Kinnser by WellSky is advantageous.
- Excellent communication skills are essential.
- Compassion and kindness are essential qualities.
Responsibilities
- Assist with ADLs (bathing, dressing, grooming, toileting).
- Support patient mobility and transfers.
- Monitor and record vital signs.
- Provide medication reminders and ensure adherence.
- Assist with meal prep and dietary monitoring.
- Maintain patient hygiene and comfort; change linens; oral care.
- Observe and report changes to nursing team.
- Document care and vital signs in EMR (Kinnser).
- Communicate clearly with patients, families, and healthcare team.
- Follow infection control and safety protocols.
- Provide transportation to appointments as needed.
- Offer emotional support and companionship.
- Collaborate with nurses and therapists for comprehensive care.
- Educate patients and families on self-care and safety.
