PeaceHealth in Springfield, Oregon is seeking a Clinical Social Worker – Behavioral Health for a full-time, day shift. The role focuses on biopsychosocial assessments, developing individualized treatment plans with the interdisciplinary team, and coordinating care transitions to ensure safe, effective discharge planning.

You’ll provide therapeutic interventions, advocate for patients and families, coordinate referrals, and support post-hospital care while upholding privacy laws and quality

Qualifications

• Master’s Degree in Social Work from an accredited graduate school of social work.
• At least 2 years of clinical experience with the designated population.
• Preferred experience in hospital, emergency room, chemical dependency, or community agency settings.
• Required credentials: LCSW (OR), CSWA (OR), or LMSW (OR).

Responsibilities

• Complete intake documentation, provide ongoing assessment, develop treatment plans in collaboration with the multidisciplinary team, manage multiple complicated cases, and maintain current knowledge of community resources.
• Monitor patient needs and progress, develop aftercare plans, and maintain documentation throughout hospitalization.
• Provide brief therapeutic interventions with patients, families, and community staff in coordination with the physician and treatment team.
• Serve as an integral member of the treatment team in the provision of therapy and aftercare planning.
• Ensure compliance with privacy and confidentiality laws and interface regularly with legal entities to address treatment needs.
• Assist with monitoring and evaluation of services provided for department quality improvement plans.
• Maintain current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, and religion, and utilize appropriate communication skills.
• Provide information, education, and direction in response to community requests.
• Access pharmaceutical and community resources to provide patients with needed psychiatric medications.
• Diagnose, assess, and address mental health and chemical dependency conditions; perform risk assessments for suicidality, homicidality, and grave disability.
• Screen and identify patients who need care management per high‑risk criteria.
• Develop, implement, and monitor a comprehensive discharge plan of care through an interdisciplinary team process.
• Perform other duties as assigned.

PeaceHealth is seeking a Clinical Social Worker – Behavioral Health for a full‑time, 1.00 FTE, day position. The salary range for this role is $46.27 – $69.41, with the hiring rate dependent upon education, training, work experience, seniority, and applicable collective bargaining agreements.

In the behavioral health unit, the clinical social worker plays a central role in patient stabilization and care transitions beyond traditional social work functions. This role includes completing comprehensive biopsychosocial assessments on all admitted patients, actively shaping individualized treatment plans in collaboration with the interdisciplinary team, and serving as the primary liaison to identified supports and community partners. The social worker coordinates legal processes, referrals, and post‑hospital care, educates families and supports on discharge plans and safety considerations, provides individualized therapeutic interventions, facilitates group therapy, and ensures continuity of care through proactive discharge planning and participation in interdisciplinary rounds.

Job Summary
