Job summary

Advocate Condell Medical Center is seeking a Licensed Physical Therapist for the Acute Care Rehabilitation unit on a part-time schedule. Must work at least one weekend day a month, two late joint shifts a month, and one holiday a year.

Responsibilities include evaluating referrals, developing and adjusting care plans, documenting per policy, and collaborating with the care team. Requires Doctorate in Physical Therapy and state licensure; BLS within 90 days.

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state of practice.
• BLS for Healthcare Providers within 90 days (AHA).

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes and implements individualized plans of care with regular outcome monitoring and updates.
• Documents evaluations, progress notes, and discharge summaries per policy and accreditation standards.
• Coordinates with physicians and team to ensure optimal patient care and safety.
• May supervise PT assistants and students as requested.
