Job summary

Qualifications
- Graduate of an ACPE-accredited School of Pharmacy.
- BS Pharmacy (minimum) and/or Doctor of Pharmacy degree from an ACPE-accredited college of Pharmacy (preferred).
- State of Michigan Pharmacist licensure - current / eligible.
- State of Michigan Controlled Substance license - current / eligible.
- Demonstrates sound professional judgment.
- Knowledgeable of software applications related to the job including Word, Excel, PowerPoint, Hospital clinical applications, Internet browser, MAPS/MedWatch/VaccineWatch.
Responsibilities
- Ensures safe, appropriate, cost-effective medication therapies for patients according to policies and standards.
- Participates in development and maintenance of clinical pharmacy services.
- Provides clinical consultation to physicians and staff and education to patients and caregivers.
- Supervises the preparation of sterile IV admixtures and chemotherapy.
- Participates in quality improvement and pharmacoeconomic efforts.
