LiveWell Homecare Agency in Missouri seeks a licensed pharmacist for a full-time daytime role focused on medication management and patient care. You will work with a multidisciplinary team to optimize therapy and ensure safe dispensing.

Qualifications

• Pharmacist license (RPh) and MO MT certification.
• Minimum of 2 years of experience.
• Bachelor’s Degree in Pharmacy.
• Doctorate in Pharmacy with 2–5 years of experience preferred.

• Assess, monitor, initiate, and modify medication use based on patient needs.
• Ensure prompt, accurate medication dispensing and resolve medication-related issues.
• Supervise supportive staff and provide education on drug therapy.

• Manage workflow, supervise technicians, interns, and volunteers, ensuring accuracy and compliance with regulations.
• Participate in departmental meetings and committees.
• Serve as a medication therapy resource, providing drug information, staff training, and transitions of care.
• Evaluate medication orders for appropriateness, documenting all activities.
• Identify opportunities to enhance efficiency and safety within medication use processes.

Qualifications & Skills

• Bachelor’s Degree in Pharmacy.
• Minimum of 2 years of experience; no supervisory experience required.
• Licensed Pharmacist (RPh) and certified in Medication Therapy Services in MO.
• Preferred: Doctorate in Pharmacy with 2-5 years of experience.
