Job summary

Qualifications

• Bachelor’s Degree in Pharmacy is required.
• Master’s degree is preferred.
• Strong communication, collaboration and clinical judgment are essential.
• Experience in medication management, compounding, and supervising pharmacy personnel is valued.

Responsibilities

• Deliver pharmaceutical care within designated patient care areas.
• Coordinate resources to ensure timely, efficient care.
• Interpret and clarify physicians’ medication orders and review patient profiles for interactions.
• Review prescribing and formulary issues, suggest alternatives based on clinical parameters.
• Consult with medical staff on drug therapy problems and monitor lab results.
• Provide drug information to healthcare professionals and medication counseling for discharged patients.
• Participate in rounds, calculate dosages, verify orders, and prepare compounded or sterile medications.
• Report supply needs, supervise pharmacy staff, and ensure adherence to standards.
• Collaborate with healthcare teams to resolve pharmacy-related issues and assist in policy revisions.
