Job Description

We are seeking Full and Part-Time Licensed Practical Nurses (LPN) for our ICF residences in Manhattan and Queens. The LPN will monitor health and safety needs of residents to promote wellness.

Locations and Schedule:

  • Manhattan: 126 East 126th Street
  • Queens: 89-02 162nd Street, Jamaica

Schedules and Compensation:

  • Full-Time Manhattan: 3 days/week (Mon/Wed/Fri 7am-7pm or 7pm-7am), $62,400/year + benefits
  • Part-Time Manhattan: Saturday overnight (7pm-7am), $33/hour
  • Full-Time Queens: Overnight Fri-Sun (7pm-7am), $62,400/year + benefits
  • Part-Time Queens: Monday 7am-7pm, $33/hour
Essential Responsibilities
  • Supervise and assist with personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness.
  • Oversee mealtime and feeding protocols, ensuring nutritional and medical adherence.
  • Administer medications as per protocols.
  • Report and manage infection control practices.
  • Maintain first aid supplies and safety protocols.
  • Document medical findings and report significant deviations to RN.
  • Communicate with families and medical providers regarding residents' health.
  • Provide post-operative and routine medical treatments as ordered.
  • Monitor vital signs, weights, and other medical information.
  • Ensure compliance with federal and state regulations.
  • Utilize proper medical charting skills.
  • Attend medical appointments and emergency procedures as needed.
  • Provide staffing coverage and complete training requirements.
  • Perform additional duties as assigned.
Qualifications
  • Current NYS LPN license required.
  • CPR certification (AHA or equivalent).
  • Experience with individuals with intellectual and developmental disabilities preferred.
  • Excellent communication skills.
  • Ability to handle multiple priorities and work as part of a team.
  • Tactful interaction with residents, families, staff, and external agencies.
  • Knowledge of program policies and procedures.
  • Valid NYS Driver's license highly preferred.
