• Experience managing services for individuals.

• Experience preparing and maintaining written narrative documentation of actions taken.
• Experience interviewing to obtain information to evaluate the next steps to take in a process.

Responsibilities

• Match the needs of Aging and People with Disabilities with available programs or resources.
• Screen applicants for long term community-based care and nursing homes.
• Assess eligibility, redetermine needs, and adjust benefits and services accordingly.
• Advocate for clients and aging and persons with disabilities within the community.
