Job summary

State of Oregon – Department of Human Services in Hillsboro seeks a Case Manager to assess, coordinate, and arrange long-term care services for older adults and people with disabilities. You will develop and monitor person-centered service plans while coordinating with providers and other agencies to ensure eligibility decisions are documented accurately.

Preference for bilingual English/Spanish; requires valid driver's license and experience in human services.

Qualifications

• Valid driver's license required.
• Three years of human services experience.
• Bachelor's degree in Behavioral/Social Science or closely related field.
• Equivalent combination of experience, education, and training.

Responsibilities

• Conduct interviews with participants to determine initial and ongoing eligibility for long-term care, healthcare, social services, and protective services.
• Document, review, and update case information and long-term care services using state systems and assessment tools.
• Assess needs, safety, and risk to develop detailed, person-centered service plans and determine appropriate services, referrals, and resources.
• Process applications, inquiries, and eligibility information within policy timeframes with accurate documentation and communication of decisions.
• Contact participants monthly to monitor service plan and safety needs.
• Collaborate with participant, other agencies, medical providers, and community partners to provide benefits, services, and information.
• Create person-centered risk mitigation plans and provide mitigation as needed.
