Job summary

Lydia's House of Hope, a program of Seeds of Faith, Inc., is seeking a highly organized, compassionate Part-Time Case Manager to support women and children in a residential transitional program in Somersworth, NH.

The role emphasizes relationship-building, coordinating services, and ensuring follow-through to promote stability and independence for residents. Strong boundaries and teamwork are essential.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred.
• Strong written, verbal, organizational, and computer skills.
• Experience in case management, residential services, or social services is preferred.
• Experience with women, children, families, homelessness, trauma, or related populations is beneficial.

Responsibilities

• Develop and maintain individualized case plans based on residents' needs, goals, and progress.
• Meet regularly with residents to review goals, challenges, appointments, services, and next steps.
• Coordinate medical, mental health, educational, childcare, transportation, employment, housing, benefits, and community resources.
• Maintain accurate resident schedules, appointments, deadlines, documentation, and case notes.
• Follow responsibilities from start to finish, including confirmations and referrals.
