Qualifications

• Experience providing targeted case management to children and families.
• Strong communication and organizational skills.
• Mentorship or leadership experience preferred.

Responsibilities

• Maintain caseload and help families access health, education, and community resources.
• Complete assessments and develop individualized service plans with members and caregivers.
• Coordinate child and family team meetings, referrals, and service transitions.
• Advocate for member choice and address barriers like transportation or waitlists.
• Support new case managers through onboarding and ongoing mentorship.
• Monitor deadlines, documentation, and approvals; ensure follow-through on referrals.
• Assist with chart reviews and quality improvement with supervisor guidance.
• Collaborate with agency leadership, schools, clinicians, and community partners.
• Maintain timely documentation in Alleva and compliance with MaineCare Section 13 requirements.
• Raise safety or service gap concerns to the appropriate supervisor.
