Job summary

Wasatch Behavioral Health is seeking a Part-Time Case Manager at the Eagle Mountain Family Clinic in Eagle Mountain, Utah. You will assist clients to access resources, coordinate services, and support families through mental health challenges. You will work alongside a multidisciplinary team to help clients build stability, confidence, and progress toward personal goals.

The role offers a competitive hourly wage of $21.36–$29.88, with DOE. Schedule is Monday–Friday with varied hours.

Qualifications

• Bachelor's degree with SSW licensure or willingness to pursue licensure.
• Valid driver's license.
• Basic understanding of Social Work principles.
• Familiarity with social welfare policy and human development.
• Ability to problem-solve and maintain a client-centered approach.

Responsibilities

• Conduct client assessments and help create individualized care plans.
• Coordinate services and collaborate with other providers to support client needs.
• Offer ongoing support, guidance, and follow-through with clients.
• Connect clients and families with community resources and supportive services.
• Maintain accurate documentation, case notes, and client records.

The Eagle Mountain Family Clinic provides a variety of therapeutic services for clients affected by trauma and/or mental illness. As a Case Manager, you'll help clients access community resources, coordinate services, and connect with the support they need while working alongside a collaborative multidisciplinary team to help clients build stability, confidence, and progress toward their personal goals.

Compensation: $21.36–$29.88 DOE and Licensure

What You'll Do as a Case Manager

• Maintain accurate documentation, case notes, and client records.

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.

• Bachelor's degree with Social Service Worker (SSW) license
• Bachelor's degree in any field with willingness to pursue SSW licensure

Additional Requirements:

• Valid driver's license
• Basic understanding of Social Work principles
• Familiarity with social welfare policy and human development
• Ability to problem-solve and maintain a client-centered approach
• Spanish speakers may qualify for increased wages after testing

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position, you must provide an official copy of licensure or certification.
