Job summary

Wilson Health seeks a part-time Case Management RN to coordinate patient care, support safe discharge planning, ensure appropriate utilization of resources, and promote positive outcomes across the continuum of care. You will collaborate with physicians, nursing, payers, and community resources to identify needs and coordinate services.

The role emphasizes utilization review, clinical documentation, and ensuring medical necessity while supporting inpatient denial prevention and management.

Qualifications

• RN licensure in Ohio is required.
• BSN preferred; Case Management experience preferred.
• Knowledge of utilization review and discharge planning.

Responsibilities

• Perform utilization review to evaluate admission and level of care.
• Assess patient needs across clinical and social domains.
• Develop and coordinate individualized discharge plans.
• Identify barriers to discharge and coordinate resources.
• Collaborate with physicians and care teams on care plans.
• Assist with documentation to support medical necessity.
• Participate in quality improvement and continuous education.
