Qualifications

• High school diploma or GED required.
• Fully proficient in both English and Spanish.
• Passion for Esperanza Health Center's Christ-centered mission.

Responsibilities

• Perform outreach to close care gaps for a panel of patients, including cancer screening and well visits.
• Coordinate with care teams and refer to Social Services or Behavioral Health as needed.
• Schedule patients for preventive care following established rules.
• Engage in cancer screening follow-up and abnormal results navigation.
• Support PrEP discussions and referrals for eligible patients.
• Assist with lab scheduling and coordinating appointments for PrEP patients.
