Job summary

Esperanza Health Center in Philadelphia seeks a Nurse Practitioner to serve as a Primary Care Provider. This full-time role delivers holistic, Christian-based care within a multidisciplinary team, emphasizing patient education, spiritual support, and wellness initiatives.

The NP will manage timely care, follow-up, and chronic disease management, while integrating faith respectfully into clinical care and partnering with community churches to serve underserved populations.

Qualifications

• Licensed Nurse Practitioner (NP); Family Medicine preferred.
• At least 2 years experience in primary care.
• Demonstrated commitment to Christian values and ethical practice.
• Passion for serving underserved populations.
• Strong communication, teamwork, and organizational skills.
• Ability to integrate faith into clinical care in a respectful, patient-centered manner.
• Accountable to and active in a Bible-teaching church congregation.
• Esperanza Health Center requires all on-site workers to be vaccinated against COVID-19.

Responsibilities

• Provide holistic, patient-centered care rooted in Christian values.
• Timely and effective clinical care, collaboration with a multidisciplinary team.
• Engage in spiritual outreach and participate in organizational wellness initiatives.
• Arrive on time, be prepared for AM/PM sessions and lead daily team huddles.
• Delegate effectively, follow up on discharges, test results, referrals, and refills.
• Complete charting and documentation on the day of visit where possible.
• Provide culturally sensitive and spiritually integrated care.
