Better Together ND in Jamestown, ND seeks a part-time Care Coordinator to empower clients through coordinated care, housing, and employment navigation. You will advocate for participants, collaborate with local agencies, and document progress to sustain recovery and well-being.

The role offers flexible hours with in-person work in the Jamestown area, and a compensation approach based on outcomes per participant, paid monthly. Experience in human services and a valid driver’s license are required.

Formación

• Bachelor's degree or four years of direct human services experience.
• Background in case management, parole/probation, or addiction services is valuable.

Responsabilidades

• Direct Engagement: conduct regular meetings with participants to monitor progress and provide guidance.
• Care Planning: assess needs and develop participant-centered plans aligned with client goals.
• Inter-Agency Collaboration: partner with local agencies and care teams to support recovery, housing, employment, and reduce recidivism.
• Documentation: maintain timely, accurate records of client interactions to ensure quality care and compliance.
