Job summary

You will handle phone calls, coordinate referrals, maintain confidentiality, and collaborate with staff and external agencies while upholding patient rights and compliance with applicable guidelines.

Responsibilities

• Patient Registration
• Coordinate and prioritize tasks, maintaining daily flow of the unit in a complex, rapidly changing environment
• Communicate with staff, families, and other departments utilizing effective communication skills
• Effectively coordinate communications involving incoming calls and e-fax
• Gather intake information and patient records
• Review referrals and verify all information
• Assist with data collection and reporting that complies with local, State and Federal guidelines
• Maintain confidentiality of all patient care information to assure rights are protected
• Maintain a good working rapport with interdepartmental personnel, as well as other external referring agencies
• Other duties as requested by supervisor
