Achievement Center of LECOM Health is seeking a Part-Time Behavioral Health Care Manager in the Ashtabula area. You will work within an interdisciplinary team to help patients access services, coordinate care, and reduce barriers to healthcare.

The role supports both behavioral health and medical needs and offers flexible scheduling. The ideal candidate holds a bachelor’s degree in a human services field and has experience with care coordination and behavioral health resources.

Qualifications

• Bachelor’s degree in social work, psychology, counseling or related human services field.
• Three years of experience in behavioral health, healthcare, human services, care coordination, or related setting preferred.
• Experience with Collaborative Care Model (CoCM) or integrated behavioral health is a plus.
• Knowledge of behavioral health and community resources.

Responsabilités

• Build supportive relationships with patients.
• Coordinate care with primary care and behavioral health providers.
• Support patient caseload in behavioral health and medical settings.
• Engage in caseload consultations with psychiatric consultants and supervision.
• Navigate healthcare systems and connect patients with resources.
• Provide brief behavioral interventions and motivational interviewing.
