The role focuses on coordinating care, navigating resources, and engaging patients in their treatment plans.

Ideal candidates will hold a bachelor’s degree in a related field and have experience with care coordination in a collaborative care model.

Qualifications

• Bachelor’s degree in social work, psychology, counseling or related human services field.
• Three years of experience in behavioral health, healthcare, human services, care coordination, or related setting preferred.
• Experience with Collaborative Care Model (CoCM) or integrated behavioral health setting is a plus.
• Knowledge of behavioral health and community resources.
• Experience working with individuals who may experience barriers to healthcare or behavioral health services.

Responsibilities

• Help patients access services and stay engaged in treatment.
• Build supportive relationships with patients.
• Coordinate care with primary care providers, behavioral health professionals, other members of the treatment team.
• Participate in regularly scheduled caseload consultations with a psychiatric consultant.
• Navigate healthcare, behavioral health, and social service systems and connect with resources.
• Conduct health and behavioral health screenings and facilitate referrals.
• Provide brief, evidence-based behavioral interventions (e.g., motivational interviewing).
• Maintain contact with patients to support treatment engagement and re-engage if needed.
• Communicate treatment recommendations to medical providers and the care team.
