Job summary

The Achievement Center of LECOM Health is seeking a Part-Time Behavioral Health Care Manager to join our collaborative healthcare team in Ashtabula, Ohio. This role focuses on connecting patients with behavioral health, medical, and community resources, coordinating care across settings, and supporting engagement in treatment.

Qualified candidates hold a Bachelor's degree in social work, psychology, or related field, with 3 years of relevant experience preferred.

Qualifications

• Bachelor’s degree in social work, psychology, counseling or related human services field.
• Three years of experience working in a behavioral health, healthcare, human services, care coordination, or related setting preferred.
• Experience working within a Collaborative Care Model (CoCM) or integrated behavioral health setting is a plus.
• Knowledge of behavioral health and community resources.

Responsibilities

• Build supportive relationships with patients
• Support the behavioral and physical healthcare needs of an assigned patient caseload.
• Coordinate care with primary care providers, behavioral health professionals, other members of the treatment team.
• Weekly psychiatric consultants, and supervision with clinical director.
• Help patients navigate healthcare, behavioral health, and social service systems and connect with appropriate resources.
• Conduct health and behavioral health screenings and facilitate referrals.
• Provide brief, evidence-based behavioral interventions, including motivational interviewing, behavioral activation, problem-solving strategies, and other appropriate approaches.
• Maintain contact with patients to support treatment engagement and re-engage individuals who may have fallen out of care.
• Participate in regularly scheduled caseload consultation with a psychiatric consultant.
• Communicate treatment recommendations to medical providers and other members of the care team.
