Job summary

Easterseals VT is seeking a Part-time Post Permanency Services Coordinator in Bennington, VT to support Vermont families formed through adoption and guardianship. You will write and implement plans of care, provide home-based services, and advocate for birth families while coordinating with clinicians and schools.

Requires a Bachelor’s degree, strong writing skills, and a valid driver’s license. Benefits include health/dental/vision, PTO, paid holidays, retirement match, and tuition assistance.

Qualifications

• Bachelor’s degree in a related field required.
• Two years of experience preferred.
• Excellent writing skills necessary.
• Valid driver’s license and reliable transportation.

Responsibilities

• Write and implement care plans approved by the consulting clinician.
• Provide home-based services covering parenting education, developmental trauma, and advocacy.
• Attend meetings and trainings including PPSP trainings and VT Consortium conferences.
• Visit client homes to implement plan requirements and support groups.
• Testify in court when subpoenaed.
